Use this form when a model is paid for a non-commercial shoot.
Non Commercial, Attribution, TFCD / TFDVD / TFP
Use this form when a model / makeup artist/ stylist is trading time for a CD / DVD / Prints
For commercial shoots the contract will be drawn up between you and the agency, usually by the agency. You'll usually sign away all rights to the images, but make sure the contract contains a clause allowing you to use the images from the publication (tearsheets) in your portfolio.
